1) This is a Form that uses English as its Language medium. Please be sure to read the form carefully and correctly, And input information accordingly. Be truthful about the information and do NOT try mislead us. 2) This Forms needs to be printed on a A4 paper in color only because our software is only able read printouts in A4 size and in color. Do NOT use grayscale or black and white color scheme for printout.
3) Form Printout should in such a way that content written on the border is ommitted or cut-out from printout areas. Fill The form with Blue Ink preferably. If such facilities are unavailable to you then you can ask our Marketing partners for it, Only One printed form is allowed for FREE of Printing Charge.
4) Counter sign each page with seal and sign, in such a way that seal and sign is placed on back of previous page and front of current page. Place seal and sign in locations that does NOT contain useful information for Us. 5) Do NOT offer bribe to company officials or Marketing Partners or any other representative of Elementalz Inc. Because doing so will only get you blacklisted and greatly reduce your chance of getting financial support anywhere else.
6) Use Capital letter for filling out the form, use clear and eligible writing for maximum clarity and avoiding any chance of miscommunication or error. Data entered should match the data on the documents shared, any mistake or discrepancy may lead to rejection of the application.

But be mindful you will have to bear expense of Travel and lodging of team should it be required.
